Pars plana vitrectomy is currently the most common technique used in retinal surgery. Although primarily involving the posterior segment of the eye it also affects the anterior segment. As this is a less published topic, the aim of this paper was to review the literature on changes in the anterior segment after uncomplicated pars plana vitrectomy using data based literature search. The conclusions are that even in otherwise ophthalmologically healthy patients, complications may occur in the anterior segment. Surgeons need to pay utmost attention in glaucoma patients and in those with endothelial insufficiency who are at highest risk of complications. He/she must also correctly plan a potential cataract surgery if not already done earlier. From the literature, most of the changes in the anterior segment after uncomplicated PPV are found to be temporary.

Aim: In this study we investigated the changes in anterior segment morphology in pseudophakia patients that underwent ultrasound biomicroscopy (UBM) after pars plana vitrectomy (PPV) operation without use of tamponade. Method: Pseudophakic patients who undergo PPV were enrolled in this prospective study between October 2012 and April 2015. Study included patients in whom intraocular tamponade was not used during PPV operation. UBM measurements were performed both before and 10 days after the operation. Anterior chamber depth (ACD) was measured using axial images of anterior segment. Trabecular meshwork-iris angle (TIA), ciliary body thickness (CBT), sclera thickness (ST), trabecular meshwork-ciliary process distance (T-CPD), iris-ciliary processes distance (I-CPD), and iris thickness (IT) were measured at temporal quadrant based on radial section images of the angle. Values measured before and after the operation were statistically compared with each other. Results: This study included 30 patients (18 females, 12 males) that underwent an operation ultrasound biomicroscopy (UBM) after pars plana vitrectomy (PPV) operation without use of tamponade. Mean age was 69.6 ± 9.1 (55 -85) years. Eighteen operations occurred on the left eye whereas twelve operations occurred on the right eye. CBT1, CBT2, CBT Max, T-CPD, and I-CPD were significantly decreased after operation when compared with the values of baseline (before) (p = 0.018, p = 0.012, p = 0.001, p = 0.033, p = 0.015, respectively). Other evaluated parameters did not show statistically significant changes after the operation (p > 0.05). Discussion: PPV results in significant changes in ciliary body morphology together with changes in anterior segment parameters in pseudophakic cases.

The first use of ultrasound biomicroscopy (UBM), which is an important method for monitoring and evaluating the anterior segment and the anterior chamber angle in ophthalmology, was made by Pavlin et al. (1) in the early 1990s. Sound energy is used in the high-frequency spectrum in UBM. With this spectrum, tissue penetration occurs to a depth of about 5 mm, which is more or less on the surface. UBM is a tool that enables the identification of not only anterior segment structures but also pathologies located in posterior segment structures such as the peripheral retina and ciliary body. It is an important imaging modality in the treatment of vitreoretinal diseases by cross-sectional imaging of the peripheral retina, pars plana, and vitreous base, which cannot be seen even via indirect ophthalmoscopy performed using a scleral buckling method.The most important advantage of UBM compared with anterior segment optical coherence tomography is that posterior chamber structures such as the ciliary body, lens zonules, and anterior choroid can also be displayed without dilating the pupils (2).In many vitreoretinal diseases, pars plana vitrectomy (PPV) is applied as the standard surgical approach. Many complications related to PPV may be encountered postoperatively. Some of these complications are related to the anterior segment.The aim of this study was to investigate by UBM postoperative changes in the morphology of the anterior segment in phakic patients in whom tamponade was not used during PPV. MethodsBetween March 2012 and October 2015, phakic patients for whom PPV was planned were included in the study prospectively. Patients on whom cataract surgery was performed along with PPV and/or gas or silicone oil was used as intraocular tamponade were excluded from the study, and 21 eyes of 21 patients were statistically included in the evaluation.The study was performed in accordance with the Helsinki Declaration. All patients who were included in the study were given information regarding the intervention and written informed consent was received. Approval of the local ethics committee was obtained for the execution of the work.
