Ultrasound diagnosis of splenic lymphoma: ROC analysis of multidimensional splenic indices

Abstract: To assess the efficacy of splenic size in the sonographic diagnosis of lymphomatous involvement of the spleen, the authors studied 31 patients with splenic lymphoma (Hodgkin's disease, 17 and non-Hodgkin's lymphoma, 14) and 218 individuals without evidence of splenic disease. All subjects were studied with both a linear and a sector transducer. The longitudinal, transverse and diagonal diameters of the spleen were measured, and two- and three-dimensional splenic indices were calculated. The analysis of the dia… Show more

“…Measurements were performed 1, 4, 8, and 12 to 18 months after OLT, or when requested by the clinical condition of the patient. Splenic DIIs and spleen size, evaluated by sonography as longitudinal diameter, 7,8 were measured. The single measurement of the longitudinal diameter was used, as it has been shown to be sufficient to obtain a correct estimation of splenic size.…”

“…The single measurement of the longitudinal diameter was used, as it has been shown to be sufficient to obtain a correct estimation of splenic size. 7,8 Complications able to impair portal resistance and disorders of the normal evolution were also recorded. The presence of acute rejection was considered as a complication impairing portal resistance only if histological degree was moderate or serious (2nd and 3rd degree).…”

“…CT and ultrasound criteria for involvement in normal‐sized spleen or liver included unexplained infiltrates or focal abnormalities of decreased attenuation. The spleen was considered enlarged on the CT scan, ultrasonography or SS‐R scan if the longitudinal diameter was greater than 130 mm (Hess et al , 1993; Munker et al , 1995).…”

“…However, DISL is difficult to detect by CT or US, even though it is the most common form of splenic lymphoma (34), because DISL can manifest as a normally appearing spleen such as that with microscopic involvement only. The most frequently used parameter for the diagnosis of splenic lymphoma is splenomegaly (18). Previous studies evaluated splenic index, spleen size, and spleen weight to assess the degree of spleen enlargement (81118).…”
