Ultrasound‐Guided Laser‐Induced Thermal Therapy of Malignant Cervical Adenopathy

Abstract: LITT ablation of malignant cervical adenopathy was considered safe and feasible. No intraoperative complications occurred. Further development of this technique applying laser energy delivery to mathematical imaging models should lead to more effective tumor palliation as an alternative to surgery.

“…Peralta et al (2010) et al recently described massive thermal injury to arteries and veins resulting in vascular collapse due to high energy density using the Nd:YAG laser powered at 30-45W for treatment of severe twintwin transfusion syndrome. This is the same energy level used by our group for head and neck thermal ablation (2,,300 J/cm 2 ) of cancer (Bublik et al, 2006;Sercarz et al, 2010).…”

“…Das Problem mit der Ausbreitung der Spülflüssigkeit ließe sich ggf. mit intern gekühlten Applikatoren [9,24], expandierbaren Applikatoren [9,22,24] oder durch andere Techniken der Hitzeapplikation wie der laserinduzierten Thermotherapie [3,25,26] umgehen. Entsprechende Vergleiche der LiTT und der RFA sind bei Malignomen der Leber und der Lunge bereits erfolgt [19].…”

This is the same energy level used by our group for head and neck thermal ablation (2,200-3,300 J/cm 2 ) of cancer (Bublik et al, 2006;Sercarz et al, 2010). In theory, the peripheral vascular collapse of arteries and veins produced by LITT will isolate blood flow to and from the tumor, thereby impeding drug (CDDP) washout to the rest of the body.…”
