Ultrasound guidance is a tool to increase procedural success and decrease complications of lumbar puncture. 2. Sonography in the transverse plane determines the position of the spinal midline while imaging in a longitudinal plane maps the lumbar spine levels to identify the most appropriate interspinous space. The intersection of these two lines establishes the needle insertion site. 3. Depth of needle insertion may be predicted by visualizing and measuring the distance to the ligamentum flavum. 4. The use of ultrasound guidance should always be considered to improve outcomes of lumbar puncture, particularly in patients with obesity or spinal landmarks that are difficult to palpate.
