Ultrasound-Guided Transrectal Placement of a Drainage Tube as Therapeutic Management of Patients with Prostatic Abscess

Abstract: Although rare, prostatic abscess is a serious condition that needs quick diagnosis and treatment. In our experience, TRUS-guided transrectal placement of a drainage tube is a feasible and safe treatment alternative for prostatic abscess; it is also easy to perform and well tolerated by the patients.

“…[16] TRUS-guided transrectal placement of a drainage tube is a feasible and safe treatment alternative for prostatic abscess; it is also easy to perform and well tolerated by the patients. [17] CT guided percutaneous drainage, along with appropriate antibiotics, is an effective approach to treat deep prostatic abscess as has been recently reported. [18] Gramnegative bacilli (chiefly E. coli) have caused about 60 to 80% of cases in recent studies.…”

“…Prostate abscesses present in patients with risk factors like prostate biopsy, other urinary track instrumentation or surgery, bladder voiding disturbances, intravenous drug abuse, diabetes, chronic renal failure, cirrhosis or other immunocompromised states 13. Prostate infections are most commonly caused by Gram-negative organisms associated with UTIs 4.…”

“…The diagnosis requires a high index of suspicion. Even though fluctuation on digital rectal examination is considered the diagnostic sign, it has low sensitivity 2 3. In fact, rectal examination for fluctuation should be avoided to prevent bacteraemia 5 8.…”

“…Several authors recommend the insertion of drainage tubes (e.g. nephrostomy tubes) in the abscess cavity for achieving better drainage (10,12). When the abscess recurs or cannot be completely evacuated, transurethral unroofing is a more appropriate approach, leading to better drainage of the abscess cavity with early recovery of the patient (7,9).…”
