Ultrasound in Prenatal Diagnostics and Its Impact on the Epidemiology of Spina Bifida in a National Cohort from Denmark with a Comparison to Sweden

Abstract: Objectives The aim of this study was to assess the incidence, the prenatal detection rate by ultrasound, and the pregnancy outcome of spina bifida (SB) in Denmark (DK) in 2008–2015 and to compare results to national data from Sweden. Methods Data were retrieved from the Danish Fetal Medicine Database, which includes International Classification of Diseases- (ICD-) 10 codes for pre- or postnatally diagnoses and pregnancy outcome. Missing data were obtained from the National Patient Register. Livebirth data with… Show more

“…Spina bifida (SB) is a congenital spinal neural tube defect (NTD) (McComb, 2015) often used synonymously with myelomeningocele (MMC) (Alriksson‐Schmidt, Josenby, Lindquist, & Westbom, 2018) involving multiple body systems (Bakaniene, Ziukiene, Vasiliauskiene, & Prasauskiene, 2018; Wagner et al., 2015). The annual incidence of SB in Sweden has decreased over the last decade to 1.3 per 10,000 newborns/year (Bodin et al., 2018) and is now considered to be a rare condition in the country. However, the total prevalence group has probably not decreased since their life expectancy has increased, thanks to the development of medical treatments.…”

“…A similar effect has been described in a developed country. [18] The need to offer feticide with late TOP has practical and emotional implications for both staff and patients, and means that regional policy guidelines restrict late TOP to patients with specific poor prognostic features. The problem is magnified by ~40% of prenatal SB diagnoses being late.…”

“…Spina bifida can be detected prenatally and more than 90% of cases will be detected before birth in countries with a national screening program for birth defects 26,27 . Currently, ultrasound is considered the gold standard screening tool for anatomic anomalies, as it is non‐invasive, inexpensive, safe and more specific than biochemical screening.…”
