Ultrasound of the Postpartum Uterus

Abstract: The ultrasound findings in the postpartum uterus will be described with a focus on differentiating normal from pathologic conditions. Imaging of the postpartum uterus will include a discussion of the normal postpartum uterus, postpartum hematomas, and retained products of conception. Clinical management and therapeutic implications based on sonographic findings will be emphasized.

“…They can be just the imaging of blood clots that do not require any treatment. Validating these findings Steinkeler et al 13 described a broad variability of normal postpartum sonographic patterns from fluid collection, hyperechogenic material. These findings are expression of blood products and should not be diagnosed as RCP.…”

“…Color Doppler can help accurately distinguish RPOC, because the presence of vascularity extending from the myometrium into the endometrium is reported to have a positive predictive value of 96%. 42 Vascularity may exhibit high-velocity, low-resistance trophoblastic spectral Doppler waveforms, although any degree of vascularity is suspicious (Fig. 11).…”

“…Bladder flap hematomas are typically located in the lower uterine segment within the cesarean section incision and are best seen with TA scans. 42 Rectus sheath hematomas occur in the lower pelvis adjacent to the rectus muscle. Because of their superficial location, they are best visualized transabdominally using a high-resolution linear transducer.…”
