We describe a case of endolymphatic stromal myosis (ESM) in which the lesion presented uniquely as recurrent frond-like cervical polyps. The lesion behaved aggressively, spreading to the peritoneum. Ultrastructurally, this tumor closely resembled proliferative phase endometrial stroma, in accord with the theory that ESM is a low-grade stromal sarcoma. The ultrastructural correlations may aid in developing future therapy, including progestagen administration for these unusual tumors.

We report two cases of endometrial stromal sarcoma of low‐grade malignancy (so called endolymphatic stromal myosis) with special emphasis on the histogenesis and mode of invasion. The microscopic and immunohistochemical features of the tumor cells resembled those of endometrial stromal cells. A three‐dimensional reconstruction study of the tumor disclosed that it grew by forming a horn‐ or crown‐like structure between and along the vessels. The tumor then protruded into the vascular lumina with a covering of endothelial cells. Immunohistochemical study showed that the vascular system was not lymphatic but venous. We therefore think that “endolymphatic stromal myosis” is a misnomer. Acta Pathol Jpn 39: 260–265, 1989.

A broad ligament tumor with the histologic features of a female adnexal tumor of probable Wolffian origin, as described by Kariminejad and Scully, is reported. Although previous cases were thought to be benign, in the present case, hepatic metastasis occurred 6 years after resection of the primary. Cytologic features by light microscopy were more anaplastic than in the original cases. Ultrastructural findings were not specific for any normal adult or fetal mesonephric tissue previously studied by electron microscopy.
