“…Nurses and patients consider self‐management support—transferring knowledge to patients or instructing self‐care activities (e.g., skincare)—to be the nurses’ responsibility in primary care teams (Barratt & Thomas, 2019; Falk‐Rafael, 2001; Luhr et al, 2019, Lundh et al, 2006; Macdonald et al, 2008; Müller‐Staub et al, 2015; Robinson et al, 2008; Stubblefield & Mutha, 2002). Some studies show that nurses and patients/families set goals together to mark progress in self‐management and disease management (Müller‐Staub et al, 2015; Robinson et al, 2008; Sefcik et al, 2018).
