Unconventional Treatment Options in Severe Asthma: An Overivew

Abstract: In Canada, asthma is the leading cause of admission in hospital. About of 80% of the death from asthma is preventable. Severe asthma is defined as a patient with persistent symptoms despite the use of adequate controller therapy, including multiple courses of oral glucocorticosteroids. However, about 10% of patients with severe asthma remain poorly controlled despite optimal treatment and these patients have the greatest morbidity and mortality. The management of refractory severe asthma remains extremely cha… Show more

“…These data suggest that HCQ might be used as an autophagy-inhibiting, anti-inflammatory agent for the management of SA. However, one study [35] failed to show the effectiveness of HCQ in patients with severe asthma. Further studies are required to investigate the potential usefulness of HCQ in the management of severe asthma considering the viral and genetic susceptibility factors that are reportedly associated with autophagy in asthma [4,5,24].…”

“…If patients with the "overlap syndrome" (asthma and COPD) are included, the numbers of people with refractory disease increases significantly [75]. Of the various novel therapies under consideration for refractory asthma [76], macrolides appear to be one of the most promising. A 2013 metaanalysis of 12 randomized, controlled trials (RCTs) of macrolides for the long term management of asthma in both adults and children found positive effects on peak expiratory flow rate (PEFRa measure of pulmonary function), asthma symptoms, asthma quality of life (AQL), and airway hyper responsiveness (AHR), but not on forced expiratory flow rate in 1 s (FEV1) [77].…”

“…Therapy was also given to a single patient with amphotericin B and budesonide 15,16,17,86 . Itraconazole is also preferred in patients not responding to oral corticosteroid or if a corticosteroid-sparing treatment approach is required 87,88 . But precaution should be taken while giving itraconazole capsule concomitantly with other drugs which show the pharmacokinetic type of interaction hence serum itraconazole level should be monitored to ascertain adequate absorption of the drug.…”
