Underdiagnoses and Undertreatment of Osteoporosis: The Battle to Be Won

Abstract: Treatments for osteoporosis should not be stopped (e.g. the "drug-holidays") in higher risk patients since the basic pathophysiology of osteoporosis continues; and, the evidence linking bisphosphonate use to causing atypical sub-trochanteric femur fractures is non-existent.

“…Although in the private sector the low rates of care for men with hip fracture may be based in part on reimbursement or attributed to higher rates of CMS denials for male patients, such mechanisms would not apply to care in the VA, where no such gatekeeping is in place. Similarly, research examining rates of osteoporosis pharmacotherapy after fracture have documented men's lower likelihood of care as compared with women, and downward trends in pharmacotherapythought to be a result of media attention to two serious, but rare side effects—osteonecrosis of the jaw and atypical femur fracture . Rates of pharmacotherapy in our study were similar to published estimates and underscore the importance of examining this downward trend in a system designed to minimize financial and other barriers to DXA and treatment.…”

“…Osteoporosis is generally underdiagnosed and undertreated despite the wide availability of effective pharmacologic treatments designed to reduce the risk of fracture [ 6 , 11 ]. In the USA, oral bisphosphonates are the most commonly prescribed initial treatments for postmenopausal women with osteoporosis [ 12 ], although intravenous (IV) bisphosphonates, raloxifene, denosumab, teriparatide, abaloparatide, and romosozumab are also approved for this indication.…”

“…While dementia, including Alzheimer's diseases, is part of the CCI calculator, it is not clear how the presence of dementia may affect hospital outcomes in older patients with hip fractures. A fracture is often the first sign of osteoporosis (OP) and is thus diagnosed clinically during post-hip fracture hospitalization [17]. Research describing LOS and acute care charges for hip fractures has mostly focused on these outcomes in trauma patients without a known pre-admission diagnosis of OP.…”
