Understanding and Measuring Symptom Burden in Heart Failure: A Concept Analysis

Abstract: Heart failure is a common, complex, and costly condition accompanied by a high degree of symptom burden. The concept of heart failure symptom burden has been used and measured inconsistently in research. To develop effective symptom management and palliation strategies, a clear conceptual understanding is needed. Rodgers’ evolutionary method of concept analysis was used to identify articles in CINAHL, Embase, Scopus, and PubMed focused on chronic heart failure. Analysis of 20 articles revealed key attributes (… Show more

“…Comprehensive symptom assessment is therefore an essential component of improving health and reducing exacerbations and hospitalizations (19) . The absence of gold standard measures (20) for symptom overload assessment in HF patients compromises the generalization of data available in the literature.…”

“…Despite the rapid growth in the symptom cluster field, previous studies have mainly targeted patients with various tumour types, 16 , 17 chronic obstructive pulmonary disease (COPD), 18 and heart failure. 19 Although some advances have been made regarding PN symptom clusters in stroke patients, 20 it is still unknown to us which kinds of stroke patients are prone to report a special PN symptom cluster. Furthermore, PN symptom studies in recent years on stroke patients have limited generalizability due to small sample sizes.…”

“…Reported inpatient mortality rate for patients admitted to hospital with heart failure in England and Wales is 9.2% with an increased rate of 39% within 12 months for those who survived to discharge (NICOR 2022). This group of people are elderly (mean age of 77.8 years on first admission) and often have multiple morbidities with high symptom burden even at rest (Stockdill et al 2019).…”
