Understanding the Concept of Pre-Clinical Autoimmunity: Prediction and Prevention of Systemic Lupus Erythematosus: Identifying Risk Factors and Developing Strategies Against Disease Development

Abstract: There is growing evidence that preceding the diagnosis or classification of systemic lupus erythematosus (SLE), patients undergo a preclinical phase of disease where markers of inflammation and autoimmunity are already present. Not surprisingly then, even though SLE management has improved over the years, many patients will already have irreversible disease-related organ damage by time they have been diagnosed with SLE. By gaining a greater understanding of the pathogenesis of preclinical SLE, we can potential… Show more

“…The boundary between pre-clinical autoimmunity and autoimmune disease is vague and depends on the degree of patient symptomatology, availability of laboratory data and appropriate recognition by the provider. For some conditions, the term ‘incomplete’ disease is used, suggesting that, although clinical findings are present, they do not fully meet diagnostic or classification criteria for ‘complete’ disease 137 .…”

“…The development and progression of SLE involves a complex interaction of genetic risk, diet, environmental influences, and immune dysregulation ( 3 , 25 ). Individuals with genetic risk alleles for SLE who are exposed to environmental risk factors during their lifetime could be more susceptible to autoimmune diseases where synergistic interactions facilitate the onset of pathogenic autoimmune responses.…”

“…In SLE, rheumatoid arthritis and type 1 diabetes (T1DM) the presence of disease-specific autoantibodies prior to clinical disease manifestation is well-established (6)(7)(8)(9)(10). This is less well characterized for autoantibodies targeting BP180 or BP230, which characterize and cause bullous pemphigoid (14).…”
