Understanding the diversity and dynamics of living with diabetes: a feasibility study focusing on the case

Abstract: Despite growing evidence about treatments, many people living with diabetes have poor diabetes control even when health care is available. One difficult issue is how to apply medical evidence to individuals. This feasibility study explores change over time and the diversity of pathways to similar health outcomes, to understand how evidence can be tailored to the individual. Six people living with diabetes (2 with type 1, 4 with type 2) agreed to a series of interviews and diary keeping. Reading the data set fo… Show more

“…A semi-structured topic guide aimed to capture the experience of retinopathy screening from the perspective of professionals and of patients (Boxes 1 and 2). To analyse the data, a comparative case study design was used13,14 to identify factors leading to high or low screening uptake, with each GP practice representing a case. Some of the authors conducted a thematic analysis of transcripts from one programme area each to extract factors influencing screening uptake.…”

“…Working within the 'swampy lowlands' 16 of general practice invokes care that is characterised by complexity, uncertainty, and change. [17][18][19] Consider a typical patient seen in today's general practice: a woman in her 80s with chronic heart and lung disease; taking 15 medicines a day; with no family, living in residential care as a result of cognitive decline and social isolation; and who recently fell and fractured her hip. Chronic disease guidelines define 'best' care for the management of each of her disease-states, but in turn contribute to the 15 medicines a day.…”

“…As we aimed to focus both on women's perceptions of specific CAMs and their overall approaches, thematic coding followed two different approaches: KJ developed codes for themes related to women's views of CAMs while AL treated each interview as a 'case', summarizing each women's approach to CAMs as a whole. This approach draws on case-based sociological approaches to complex interactions (Griffiths, 2009;Miles & Huberman, 1994) and has successfully been used by members of the research team to understand and contextualize complex individual dynamics such as living with chronic illness over time (Griffiths, Manazar, Chow, Anto et al, 2007). Our approach assumes that women's experiences and choices can best be explained not by isolating individual factors (e.g.…”
