Understanding the Economic Impact of an Essential Service: Applying Time-Driven Activity-Based Costing to the Hospital Airway Response Team

Abstract: BACKGROUND: As the United States moves toward value-based care metrics, it will become essential for anesthesia groups nationwide to understand the costs of their services. Time-driven activity-based costing (TDABC) estimates the amount of time it takes to perform a clinical activity by dividing complex tasks into process steps and mapping each step and has historically been used to estimate the costs of various health care services. TDABC is a tool that can be adapted for variable staffing models and the volu… Show more

“…Stone et al 27 examined a hospital airways response team responding to a limited number of rapid response scenarios in a tertiary US setting comparing the cost of running the team and bills for call-outs to insurers as their key metric. In this setting, the authors concluded that ‘what is billable and non-billable may not reflect either the need for or the cost of providing the service’.…”

“…In the operating room, this process is rather simple, but for activities such as airway response teams and preoperative clinics, this is a more complicated process. Stone 19 presents a time-driven activity-based costing (TDABC) model to estimate the cost incurred by an anesthesiology department. The authors conduct a detailed analysis and conclude (not surprisingly) that anesthesiologists provide hospitals with a service that may be undervalued using traditional top-down costing methods.…”

“…The paper titled "Applying Time-Driven Activity-Based Costing to the Hospital Airway Response Team" is particularly relevant during the COVID-19 pandemic. 19 Much has been written on the acute increase in the workload involving anesthesiologists and, in particular, their paramount role in the airway response teams. As we move more toward value-based care, it is essential for anesthesia practices to understand the costs of their services.…”
