Unilateral Extravesical Ureteral Reimplantation in Children Performed on an Outpatient Basis

Abstract: Extravesical ureteral reimplantation for unilateral VUR may be performed without compromise in quality on an outpatient basis with significantly less use of intravenous analgesics and anti-emetics.

“…The latter is associated with increased pain and spasm and therefore medication requirements due to extensive trigonal dissection, need to open the bladder and extended postoperative catheterization [10,14]. These problems are avoided in the EVR, which leads to greatly reduced patient discomfort postoperatively and allows the procedure to be performed in an outpatient setting [6,7,10]. In fact, the peri-operative outcomes for the minimally invasive ureteral reimplant are more appropriately compared to the endoscopic anti-reflux procedures, whose long-term reflux resolution rates remain problematic [13].…”

“…Since the advocacy for the ureteral advancing suture and detrussorhaphy [4] several groups have further advanced this technique performing it as an outpatient procedure [5,6], and bilaterally [7]. Traditionally, the technique has been performed through a standard Pfannenstiel incision which could hardly be labeled as minimally invasive.…”

“…Extravesical ureteral reimplantation has shown similar success rates to the open procedure. 2,4 The extravesical procedure has traditionally been performed for unilateral VUR because bilateral reimplantation has been associated with transient urinary retention, requiring bladder catheterization. We have reported a modification of the surgical technique and a critical pathway that has resulted in same-day patient discharge of both unilateral and bilateral reimplantation procedures, without increased morbidity, urinary retention, or hospital readmissions.…”
