Unilateral Internal Cerebral Vein Thrombosis: Case Reports and Review of the Literature

Abstract: The purpose of this manuscript is to present two cases of unilateral internal cerebral vein thrombosis (UICVT) and use them to review the typical imaging findings and clinical presentation, provide a differential diagnosis when presented with a unilateral thalamic lesion, and discuss current literature findings of internal cerebral vein thrombosis, including the interesting trend of left-sided involvement. Two cases of UICVT were gathered from our institutions. A literature review was then conducted, and cases… Show more

“…Some of the bilateral thalamic infarcts, as established on imaging might have been isolated right thalamic afflictions initially 6. The presence of smaller collateral venous channels draining the right thalamus is found more often on the right side in the setting of bilateral internal CVT 7 8. This may mirror the well-known asymmetry where the superior sagittal sinus drains preferentially into the right transverse sinus 9.…”

“…The vein of Galen and straight sinus thrombosis typically show bilateral thalamic lesions [ 3 ]. However, very rarely, the vein of Galen and straight sinus thrombosis shows a unilateral deep cerebral lesion, especially on the left side [ 6 , 7 ]. Although the asymmetry of the venous outflow tract may be speculated as one possible mechanism [ 7 ], imaging has not confirmed its mechanism.…”

In the present case, the left BVR showed higher congestion than the right.…”
