Unilateral Parathyroidectomy in Hyperparathyroidism Due to Single Adenoma

Abstract: As a general principle in the treatment of primary hyperparathyroidism due to single adenoma, unilateral parathyroidectomy was applied to 50 patients and compared with another group of 50 conventionally explored patients. Twenty-five patients were explored only on the "adenoma" side. The other 25 patients were explored on both sides, avoiding biopsies at the first. In the conventionally explored patients, the adenoma was removed and one to three normal glands were biopsied. Oil-red-O technique was used in the … Show more

“…PHPT'nin en sık nedeni olan tek adenomlarda gittikçe artan biçimde tercih edilen cerrahi yöntem, ilk kez Tibblin tarafından tanımlanan MİP'dir. 4 MİP tekniğinin cerrahi kür başarısını etkileyen en önemli faktör, preoperatif görüntüleme tekniklerinin etkin kullanımıdır. Lokalizasyon için en çok tercih edilen görüntüleme teknikleri USG ile Tc-99m MIBI sintigrafisidir.…”

“…For these reasons, parathyroid surgery has progressively moved towards some type of limited neck exploration. The first attempt was made in the early 1980s by Tibblin et al (5) who proposed a unilateral neck exploration (UNE) based on parathyroid adenoma removal and biopsy of the ipsilateral parathyroid gland to discover possible hyperplasia. In the 1990s UNE evolved towards even more limited surgical approaches: the endoscopic (6) and the radio-guided (7) minimally invasive parathyroidectomy.…”

“…In 1986, John L Doppman stated 'In my opinion, the only localizing study indicated in a patient with untreated primary hyperparathyroidism (pHPT) is to localize an experienced parathyroid surgeon'. At the same period, some surgeons had developed unilateral parathyroidectomy due to the high frequency of single adenomas (1). The improved performance of radiological and functional investigations has enabled the development of image-guided focused approaches.…”
