Unilateral pulmonary artery agenesis presenting with hemoptysis: Treatment by embolization of systemic collaterals

Abstract: Systemic-to-pulmonary collateral vessels and shunts develop in patients with isolated unilateral absence of a pulmonary artery (IUAPA). Two cases of IUAPA (right and left) with hemoptysis in adult life are presented. Selective embolization of these systemic vessels controlled hemoptysis successfully. These cases represent an example of an alternative treatment for this rare anomaly.

“…Although pneumonectomy with ligation of collaterals or reanastomosis of the peripheral pulmonary artery with the main pulmonary artery is a more definitive treatment, these major operations carry significant risks and may not be feasible in the emergent setting. Transcatheter embolization of aortopulmonary collaterals, either alone or as an adjunct to surgery, has shown to be a safe and effective treatment of hemoptysis related to PIPA in a small number of case reports ( 5 , 6 , 7 ).…”

“…Patients who develop pulmonary hypertension can be treated medically with vasodilator therapy [ 11 , 12 ]. Hemoptysis may be self-limiting, selective embolization of the systemic collaterals and in selected cases pneumonectomy of the affected side may be required for massive hemoptysis [ 13 , 14 ].…”

“…Treatment options of UPAA involve embolization of the abnormal systemic to pulmonary collateral vessels in cases presenting with hemoptysis, and even in few cases, pneumonectomy of the affected lung may even be essential. [ 1 10 ]…”
