Unilateral Renal Vein Thrombosis with Nephrotic Syndrome

Abstract: Medical Memoranda MEDICURNAL 1523 attacks, occurring between 8.30 and 9 a.m., were noted to follow runs of coupled ectopic beats, and recurrent ventricular fibrillation was shown by E.C.G. (Fig. 1). Each attack either terminated spontaneously or was terminated by external cardiac massage, but the basic dysrhythmia of coupled ventricular ectopic beats persisted.

“…In 30,000 autopsies performed in the Mayo Clinic between 1920 and 1961, only 16 adult cases were found, (0, 06%), and only 2 patients had a previous clinical history of nephrotic syndrome. It is interesting to mention that the renal biopsies of the non occluded side were normal in the cases reported by Khokhani (8), whereas in the cases reported by Rosenmann (5) and in our own cases the abnormal histological patterns were found in both kidneys whether they had the obstruction or not. More recently Rosenmann and co-workers (5) have studied 15 cases.…”

“…Thrombosis of the renal veins is a very rare condition. In this case and in other cases reported later by Richet et al (7), (1965), Khokhani et al (8), (1966), and Rosenmann et al (5), (1968), the diagnoses were made by selective catheterization of the renal veins through the vena cava or surgical and necropsy findings. McCarthy (4) in 1963 found 38 cases in the world literature of nephrotic syndrome and renal vein thrombosis 2 of them being personal observations.…”

“…Since then thrombotic complications have frequently been observed in nephrotic patients in both venous and arterial systems [14,15,31,39,45,50]. The association of the nephrotic syndrome with renal vein thrombosis has been found to be particularly frequent [23,26,33,39,45] and it is now generally considered to be a complication, while in the past was regarded as a cause of the nephrotic syndrome [30,32], The tendency of nephrotic patients to thrombotic episodes has been attributed to a hypercoagulable state. Therefore, some aspects of haemostasis in such patients have been studied previously [9,22,25,53], but no complete haemostasis data are available at present.…”
