Universal coverage but unmet need: National and regional estimates of attrition across the diabetes care continuum in Thailand

Abstract: Background Diabetes is a growing challenge in Thailand. Data to assess health system response to diabetes is scarce. We assessed what factors influence diabetes care cascade retention, under universal health coverage. Methods We conducted a cross-sectional analysis of the 2014 Thai National Health Examination Survey. Diabetes was defined as fasting plasma glucose 126mg/dL or on treatment. National and regional care cascades were constructed across screening, diagnosis, treatment, and control. Unmet need was d… Show more

“…Even though Indonesia has implemented universal health coverage as mentioned earlier, critical challenges in providing equitable access, ensuring quality services and financial protection for people still persist (Ardiana, Ekoriano & Fathonah, 2019). Similar findings about universal health coverage from the health perspective have been reported from Thailand and even Europe (Reeves, Mckee & Stuckler, 2015;Yan et al, 2020).…”

“…In cases in which older participants could not communicate or had cognitive problems, information was collected from their informant caregivers. The details of this survey are described elsewhere [25]. A total of 23,760 individuals were invited to participate in the NHES-V, among whom 8,640 were aged 60 years and older.…”

“…This adjustment accounted for clustering and weighting of older participants by using a set of Svy commands in STATA [30]. This study used weighting based on the sampling probability against the 2014 registered Thai population [25]. Chi-squared tests were used to compare the background characteristics of participants who were excluded from the analysis with those of the final sample.…”

“…NCD care continua are theoretical frameworks used to monitor public health system performance by measuring steps of screening, diagnosis, treatment, and ultimately control of chronic diseases. 17 , 18 They are useful to benchmark the coverage or unmet need of primary and secondary prevention interventions for BP in a community. While population level estimates of HTN for Haiti have been reported in the 2016–2017 Demographic Health Survey (DHS), the survey focuses only on adults 35–64 and may not be representative of the wider population.…”
