Unruptured aneurysm of the sinus of Valsalva presenting with ventricular tachycardia

Abstract: We report a case of a large unruptured aneurysm of the right sinus of Valsalva which caused severe right ventricular outflow tract obstruction and presented dramatically with a life threatening ventricular tachycardia. Despite severe aortic incompetence prior to surgery it was possible to resect the aneurysm and repair the aortic root without resort to aortic valve replacement and thus return the anatomy to normal.

“…Significant arrhythmias may also be caused by unruptured aneurysms, including ventricular tachycardia [34], atrialfibrillation [35],and complete heartblock through extension into the interventricularseptum and compression of the AV node and His bundle [36]. …”

“…In the presence of complications such as right ventricular outflow tract obstruction, arrhythmias or infection, surgical repair is indicated[ 34, 36, 53]. It is generally recommended to repair large SVA to avoid later complications of rupture, myocardial ischemia, and right ventricular outflow tract obstruction.…”

“…Although outflow tract VT is commonly referred to as "normal heart ventricular tachycardia" because of the uniformly preserved biventricular size and function, subtle structural abnormalities such as congenital outflow tract, basal LV, or aortic sinus of valsalva aneurysms have been described and implicated in arrhythmia pathogenesis [9]- [12]. In the case previously described by Reithman et al, a LV basal aneurysm was identified in the area adjacent to the critical septal isthmus utilized by both tachycardias [6].…”
