Unusual angiographic manifestations of craniopharyngioma: A case report

Abstract: A craniopharyngioma with two unusual manifestations, a dense capsular stain and occlusion of the internal carotid artery, is reported. Neither of these findings appear to have been reported previously. Explanations for these unusual phenomena are offered.

Wallenberg's syndrome is the clinical manifestation of the obliteration of arterial blood vessels supplying the dorsolateral part of the medulla oblongata, the posterior inferior cerebellar artery being involved in most cases. A patient is described in whom the typical features of Wallenberg's syndrome correlated with cystic necrosis in the perfusion area of the posterior inferior cerebellar artery. A craniopharyngioma spreading along the pons and involving the posterior inferior cerebellar artery, the basilar artery, and its branches was found to be responsible for the infarction of the dorsolateral medulla oblongata.

The radiological manifestations of 16 pituitary adenomas, six meningiomas and 14 craniopharyngiomas were reviewed. The differential diagnosis of these tumors is possible with a few exceptions when skull radiography, CT, angiography and metrizamide cisternography are carefully evaluated. Prolonged injection angiography (PIA) is particularly important for demonstrating the contours of pituitary adenomas and meningiomas. Metrizamide cisternography is indicated when the tumor contours are obscure using PIA, or when the differential diagnosis is uncertain.

Craniopharyngiomas with unusual topography and associated with vascular pathology

Out of 155 cases of craniopharyngioma seen in the past 47 years, 19 are considered unusual. These 19 cases have been placed under the following headings--1. unusual topography, and 2. associated vascular pathology. Since CT scanning offers important information about extension of craniopharyngiomas, it is very helpful for planning operative approach. However, cerebral angiography is still important for demonstrating vascular pathology.
