Unusual localisation of tophaceous gout

Abstract: We report four cases with very unusual manifestations of tophaceous gout. All patients were male, aged between 29 and 67 years. Only one patient had an acute gouty arthritis in his medical history, whereas the other three had never developed clinical symptoms from their hyperuricemia. Two patients had gout tophi in the patella, while the others showed gout tophi in the space of bipartite bones, one in a bipartite patella and one in a tripartite sesamoid bone of the first toe. “…However, other presentations in different joints have been previously reported as secondary gouty arthritis to other disorders. For example, Reber et al reported a case where they discovered the presence of tophi in the patella [7], while Jacobs et al reported another case of a 77-year-old male with gout in the wrist most likely as a cause of medications [8]. In comparison to our case, the patient was medically free and was not taking any kind of over the counter drugs.…”

“…However, painful bipartite patella is occasionally observed in adolescents and young adults, especially those who regularly participate in strenuous sports [1,3,8,12,[16][17][18][19]25]. In adults, this condition sometimes becomes painful following either minor [4,10,21] or major injury, such as acutely displaced fracture of the bipartite patella [5,11] or in patients with gout tophus in the bipartite patella [13,14,22,24]. Although most patients usually respond to conservative treatment, including rest and restriction of sports activities, surgical treatment is occasionally considered for patients who fail to respond to conservative treatments.…”

“…There are a multitude of joints that can be impacted in patients with gout. More specifically, patients commonly develop tophaceous gout in cartilage, articular areas, sheaths of tendons, within subcutaneous layers, and in synovial spaces [5] . Gout presenting in the acromioclavicular joint is relatively rare.…”
