Update on the Role of Allergy in Pediatric Functional Abdominal Pain Disorders: A Clinical Perspective

Abstract: Both functional abdominal pain disorders (FAPDs) and food allergies are relatively common in children and adolescents, and most studies report an association between FAPDs and allergic conditions. FAPDs share pathophysiologic processes with allergies, including both immune and psychological processes interacting with the microbiome. No conclusive data are implicating IgE-mediated reactions to foods in FAPDs; however, there may be patients who have IgE reactions localized to the gastrointestinal mucosa without … Show more

“…Allergic reactions to food could occur through a variety of mechanisms, including IgE-mediated and non-IgE-mediated. 27 Fish allergy is usually an IgE-mediated type I hypersensitivity reaction. 28 There are two phases in allergic reactions: the sensitizing phase and the effector phase.…”

“…IBS shares pathophysiologic features with allergies, including both immune and psychological patterns, even though the mechanism through which allergens might play a role in the determinism of IBS remains unclear. In the absence of systemic allergic symptoms or oral allergy syndrome, allergic triggers for IBS are unlikely to be identified by standard testing [ 107 ].…”

“…An additional requirement for all FAPDs is that the symptoms are not, after careful evaluation, fully explained by any other medical condition [4]. The pathophysiology of FAPDs is obscure and considered multifactorial [5][6][7].…”
