Updates in Pathology for Retroperitoneal Soft Tissue Sarcoma

Abstract: Retroperitoneal tumors are extremely rare. More than 70% of primary retroperitoneal soft tissue tumors are malignant. The most common sarcomas in the retroperitoneum include liposarcomas and leiomyosarcoma, however other sarcomas, along with benign mesenchymal tumors, can occur. Sarcomas are a heterogenous group of tumors with overlapping microscopic features, posing a diagnostic challenge for the pathologist. Correct tumor classification has become important for prognostication and the evolving targeted thera… Show more

“…This option almost always requires a further surgical incision for harvesting. In addition, a large discrepancy in the diameters of these veins increases technical difficulties and can lead to a higher risk of thrombosis 16 . Techniques such as the spiral saphenous vein graft (SSVG) and the cylindrical superficial femoral vein (SFV) panel graft have been developed to overcome the latter problem, that is, to increase the diameter of the graft 17–19 .…”

“…Leiomyosarcoma (LMS) accounts for approximately one quarter of retroperitoneal sarcomas and exceptionally may arise as a primary intra-adrenal tumor, putatively of vascular wall origin. 18,19 The shared attributes of smooth muscle neoplasms and PEComa create a diagnostic challenge that has generated much commentary. 20,21 In particular, a spindle cell-rich PEComa with variably eosinophilic cytoplasm may closely simulate a smooth muscle neoplasm.…”

“…dedifferentiated, mixed, and pleomorphic. 10,11 Welldifferentiated liposarcomas are characterized as slowly growing tumors that do not have metastatic potential and have a low local recurrence rate. As a result, they have excellent clinical outcomes following a successful complete excision.…”
