Updates on ankylosing spondylitis: pathogenesis and therapeutic agents

Se Hee Kim,
Sang-Hoon Lee

Abstract: Ankylosing spondylitis (AS) is an autoinflammatory disease that manifests with the unique feature of enthesitis. Gut microbiota, HLA-B*27, and biomechanical stress mutually influence and interact resulting in setting off a flame of inflammation. In the HLA-B*27 positive group, dysbiosis in the gut environment disrupts the barrier to exogenous bacteria or viruses. Additionally, biomechanical stress induces inflammation through enthesial resident or gut-origin immune cells. On this basis, innate and adaptive imm… Show more

“…The sacroiliac joint characteristics of patients with AS undergo significant heterogeneity compared to healthy individuals, and the clinical pathogenesis is characterized by physiologic pain and stiffness in the lumbar, hip, and back [1]. As the disease worsens, the spinal joints of AS patients may be destroyed, and in severe cases, deformities of the spine may occur, thus limiting the patient's physical activities [2].…”

“…The serum levels of IL-23 and IL-17 in the peripheral blood of AS patients are markedly elevated, which is linked to the disease [ 19 , 20 ]. T cells, B cells, NK cells, and macrophages, as well as the IL-23/IL-17 axis, are all involved in the inflammatory development of AS [ 21 23 ]. Furthermore, results from other research have revealed that antibodies can be used as biomarkers to aid in the diagnosis and prognosis of ankylosing spondylitis.…”

“…Rheumatic diseases, mainly including rheumatoid arthritis (RA), ankylosing spondylitis (AS), and psoriatic arthritis (PsA), are characterized by joint involvement, chronic pain, and other systemic manifestations. [1–3] Since rheumatic diseases probably cause disability, incapacity of work, and reduction of quality of life, they remain a great challenge to global health. [4,5] Tumor necrosis factor-alpha inhibitors (TNFis), conforming to the treatment purpose of symptom relief and progression restraint, possess confirmed efficacy and are commonly recommended for the management of rheumatic diseases.…”
