The aim of this study was to investigate the effects of slow maxillary expansion (SME) on the dentoalveolar, skeletal, upper airway, and maxillary sinus using cone-beam computed tomography (CBCT). Twenty-three orthodontic patients (mean age 8.93 ± 1.61 years) who were treated with maxillary expansion using banded hyrax in the Department of Pediatric Dentistry at Jeonbuk National University Dental Hospital were included. According to the expansion speed applied, they were divided into two groups: SME (12 subjects, mean age 8.92 ± 1.45 years) and rapid maxillary expansion (RME, 11 subjects, mean age 8.94 ± 1.84 years). CBCT were obtained before (T0) and after (T1) the treatment and were analyzed with InVivo5 software (Anatomage, San Jose, CA, USA). Descriptive statistics showed no significant differences between the two groups in age, sex, or skeletal maturity. There were significant increases in maxillary width at the dentoalveolar and skeletal levels for both groups. Upper airway volume revealed a significant increase of 38.59% in the SME group and 28.72% in the RME group. However, there was no significant difference between SME group and RME group in all measurements. This study suggested the efficacy of SME in growing patients. SME was effective in increasing not only dentoalveolar and skeletal measurements but also airway volume. Therefore, pediatric dentists should select an appropriate expansion method considering the physiological aspects of periodontal tissues and discomfort in growing children.

Computational fluid dynamics (CFD) analysis is useful for quantitative assessment in patients with upper airway obstructions. We compared CFD analysis with rhinomanometry (RM) and acoustic rhinometry (AR) . Twenty patients with nasal and paranasal diseases who required computed tomography assessment underwent RM and AR. We measured the pressure and velocity at four parts of the upper airway using CFD analysis. Then we evaluated the correlation among CFD analysis, RM, and AR. CFD analysis detected obstruction sites in the nasal airway and pharynx in 14 and 2 patients, respectively. High negative pressure accompanied the nasal obstruction, even behind the nasal cavity. Nasal airway pressure measured using CFD analysis strongly correlated with nasal resistance in RM (Spearman correlation coefficient ＝ 0.853) . CFD analysisʼs sensitivity and specificity to detect the obstruction were 84.6％ and 57.1％, respectively (compared to those of RM) and 83.3％ and 50.0％, respectively (compared to those of AR) . The CFD analysisʼs ability to detect obstruction was comparable to that of RM and AR; therefore, it may help evaluate the upper airways in patients with nasal and paranasal diseases. We found impaired nasal ventilation also affected other parts of the upper airway. Further studies with a larger sample size are required to validate the use of CFD analysis for assessing the degree of upper airway ventilation disorders.

Social distancing is especially important for those patients, because children with cleft palate often present upper airway obstruction and/or sleep disordered breathing [35]. Patients with syndromes that present this trait, as Pierre Robin sequence, should also consider those recommendations, due to common respiratory disturbances, like airway obstruction that is life threatening for neonates with this syndrome [36].…”

The coronavirus disease (COVID-19) pandemic has seriously affected the global population and has raised concerns about the effect of this new disease on individuals with genetic disorders, many of which have a systemic effect, including impairment of the respiratory system. A lack of published literature about the consequences of SARS-CoV-2 infection in genetic disorders is observed, in this sense, the present review analyses the current knowledge about COVID-19 and its impacts on patients with genetic disorders, also genetic variants as factors of susceptibility and possible risks and preventive measures to those patients, highlighting the role of the restriction to social contact in prevention.
