Upper Gastrointestinal Bleed due to Duodenal Metastases of Lung Adenocarcinoma: Report of Two Cases and Review of Literature

Abstract: Upper gastrointestinal bleeding as a result of gastrointestinal metastases from lung cancer is extremely rare. We report two cases of patients with duodenal metastases from lung adenocarcinoma presented with recurrent melena. Histopathological examination and immunohistochemical staining of the duodenal biopsies supported the diagnosis of metastatic lung adenocarcinoma.

“…This agrees with Wang et al 50 . A relatively normal PAS staining intensity, similar to the control, was observed in silymarin, AFPD and EFPD treated groups and this is in tandem with Adil et al 51 , Kosasih et al 52 and Amber et al 18 who reported that the nephroprotective potential of plants extracts could be attributed to its phytoconstituents such as polyphenolic compounds.…”

“…GIT metastasis from primary lung carcinoma are rare with an incidence ranging from 4.7 to 14% in autopsy studies [1,2]. This incidence is much lower when patients are alive ranging from 0,3 to 1,8% [3,4] because of the asymptomatic course of the disease most often [5]. The existence of a table involving simple acute abdominal pain without perforation, as found in our case, remains exceptional [7,8].…”

“…Ito, et al, demonstrated that chemo-radiotherapy with four cycles of cisplatin and etoposide, followed by abdominal irradiation at a dose of 30 Gy to patients with small cell lung cancer with GI metastasis have reasonably good partial response. Zhou, et al, highlighted the potential benefit of tyrosine kinase inhibitors [TKI] in primary lung cancer with driver gene mutations in gastrointestinal metastases [4]. In addition, targeted vascular therapies have gained acceptance in recent years.…”

“…Small bowel metastasis has a relatively worse prognosis as it often leads to perforation [4,8]. Table 1 shows other presentations of adenocarcinoma that have a similar pathologic diagnosis to our patient's case [9][10][11][12][13][14][15][16]. Early detection, diagnosis, and treatment are crucial for improving patient outcomes in duodenal metastasis from primary lung cancer, which often goes unnoticed due to its benign clinical presentation, low incidence, and diagnostic challenges.…”
