Ureteric and urethral stenosis: A complication of BK virus infection in a pediatric renal transplant patient

Abstract: Viral infections remain an Achilles heel in solid organ transplant. In recent years, incidence of BK virus infection in the kidney transplant population is on rise. BK virus is known to cause severe renal dysfunction, ureteric stenosis, and hemorrhagic cystitis in renal transplant patients. Most of the reviews and prospective studies on BKV nephropathy pertain to the adult population. Although ureteral stenosis is known to occur in BK infection, urethral stenosis is not being reported in the literature. In thi… Show more

“…In immunocompromised patients, such as kidney transplant recipients, BKV can reactivate and lead to ureteric stenosis, hemorrhagic cystitis, transient renal dysfunction, and progressive renal impairment due to BKV-associated nephropathy (BKVN) [1,2]. Of these, BKVN is the most common and the most clinically significant because of its association with allograft dysfunction and the loss of renal grafts in renal transplant recipients [3].…”

“…In paediatric recipients, PUV were found to be associated with ureteric stenosis after RTX, possibly because remodelling of collagen I/III also affects the ureteric anastomosis site of the transplant ureter in these children [18]. A very rare cause of ureteric stenosis in paediatric recipients might be a BK virus infection; to date, in one case report this was found to be the underlying cause [22]. In the recipients at our centre, regular controls for BK virus in the peri- and postoperative phase of RTX are part of the routine assessments.…”

“…We read with great interest the paper by Rajpoot et al (1) recently published in your journal. They report a case of BK virus-induced allograft nephropathy (BKVAN) associated with ureteral stenosis and urethral stricture.…”
