Urethral Valves1

Abstract: The primary lesion of posterior urethral valves is a simple one which might be expected to respond readily to simple transurethral resection. Nevertheless, the total care of a boy with urethral valves can be a most complicated undertaking. The object ofthis paper is to describe a new method of valve ablation and to identify and discuss the factors leading to failure of recovery and to persistent urinary infection. From 1951 up to October 1972 boys with posterior urethral valves have been treated by the senior … Show more

“…Before the prenatal detection of urinary tract abnormalities, the majority of boys with PUV were identified in the 1st year of life with a combination of symptoms and signs such as poor urinary stream, distended bladder, failure to thrive and urinary tract infection [19,20]. These children frequently presented with septicemia, acidosis and uremia [21].…”

“…Comparing the series of valve cases reviewed in the literature, we found quite different, inhomogeneous pa tient groups, ranging from a series with a high percentage of infant patients [16] to those with a high incidence of adult patients [8]. Some authors describe milder forms of the problem presenting in later childhood in the majority of their patients [4].…”

“…Some authors describe milder forms of the problem presenting in later childhood in the majority of their patients [4]. Whereas to others, valvular obstruc tion is considered the severest cause of obstruction in infants [16]. Some reports are based on post-mortem examination [11], others on radiologic examination in patients during their stay in hospital [8,16], These series are not entirely comparable and it is therefore quite diffi cult to determine the true incidence of posterior urethral valves.…”

“…It can be performed in the first days after birth without open operation. A similar method using Fogarty's catheter has been applied in 3 newborn children by Williams (14) with good results. However, he has recently used a special hook electrode which he inserts in the anterior flap of the valve.…”

“…The removal of valves themselves, particularly in newborn boys and in fants, is the most important procedure besides biochemical correction, treatment of infection as well as temporary relief of obstruction (3)(4)(5)14).…”
