Application of laser & photonics in dentistry and medicine has a long history, however, it is still emerging as a new field enabling the application of advanced technology at precise size and levels and the exploitation of novel properties. Many areas of dental/medical device are expected to benefit from photonics including sensors for use in the laboratory , the clinic, and within the human body. For example, the potential uses of laser include the early detection and treatment of disease. These new innovative biomedical pho-tonics include "advanced bioimaging" to push photon-based imaging tools to new limits of resolution and utility for "molecular and cellular imaging" to develop and apply advanced optical diagnostic probes to important problems in molecular and cellular biology, and "biomedical photon-ics" to translate photonics to clinical dentistry/medicine for disease screening, diagnosis and therapy. Launched in April 2015, the Japan Agency for Medical Research and Development （AMED） pursues its assigned mission to fast-track medical research and development in support of humanity. AMED is currently tackling the challenge of reforming Japan＇s funding allocation and evaluation systems. Other significant tasks in fast-tracking the social implementation of research outcomes include rapid diagnosis of undiagnosed diseases, and novel therapeutic interventions for patients. AMED engages in research and development in the field of medicine, establishing and maintaining an environment for this R&D, and providing funding, in order to promote integrated medical R&D from basic research to practical applications, to smoothly achieve application of outcomes, and to achieve comprehensive and effective establishment / maintenance of an environment for medical R&D. Providing a one-stop service for research expenses, AMED consolidates budgets for research expenses, which had previously been allocated from different sources-the Ministry of Education, Culture, Sports, Science and Technology , the Ministry of Health, Labour and Welfare, and the Ministry of Economy, Trade and Industry. AMED aims to achieve the world＇s highest level of medical care / service and to form a society in which people live long, healthy lives by promoting integrated research and development, from basic research to practical application, and by establishing and maintaining an environment therefor, and linking this to various forms of growth in medical R&D. In this special lecture I would introduce a birds-eye view not only AMED supporting R&D program but also FMDIPA activity to develop dental/medical laser device industry, currently being undertaken to overcome the problems facing dental/medical diseases. He has made extensive contributions to the development of the bioengineering profession worldwide through his extensive professional activities and service with the government. Objective. Second-trimester sonographic findings of fetal trisomy may include structural abnormalities or sonographic markers of fetal aneuploidy. Unlike structural anomalies, sonographic markers of fetal aneuploidy are insignificant by themselves with regard to outcome, are nonspecific-most frequently seen in normal fetuses, and are often transient. Our objective was to review the secondtrimester sonographic findings of the major trisomic conditions, trisomies 13, 18, and 21. Methods. We reviewed a number of the most commonly accepted markers, including nuchal thickening, hyperechoic bowel, echogenic intracardiac focus, renal pyelectasis, shortened extremities, mild cerebral ventricular dilatation, and choroid plexus cysts. Markers associated with trisomy 21 were emphasized. Results. The sensitivity of sonography for detection of fetal trisomic conditions varies with the type of chromosome abnormality, gestational age at the time of sonography, reasons for referral, criteria for positive sonographic findings, and the quality of the sonography. As an estimate, 1 or more sonographic findings can be identified in approximately 90% of fetuses with trisomy 13, 80% of fetuses with trisomy 18, and 50% to 70% of fetuses with trisomy 21 (Down syndrome). Conclusions. The presence or absence of sonographic markers can substantially modify the risk of fetal Down syndrome and is the basis of the so-called genetic sonogram. Because maternal biochemical and sonographic markers are largely independent, combined risk estimates will result in even higher detection rates than either alone.

There have been a significant number of reports on the clinical utility of measurement of ‘hyperglycosylated’ isoforms of the pregnancy hormone, human chorionic gonadotropin (hCG). Although there are a variety of hCG isoforms which can be termed ‘hyperglycosylated’, the measurements were all made using a unique antibody designated B152. This antibody was raised using a choriocarcinoma-derived form of hCG, which was hyperglycosylated with N- and O-glycans and was also 100% ‘nicked’ hCG. Antibody B152 was recently mapped to a linear epitope around a single O-glycan on the β-subunit of hCG at residue number 132. Thus, the antibody can only measure isoforms of hCG that possess a core 2 type of branched O-glycan on this portion of the hCG β-subunit. Isoforms that are hyperglycosylated in the hCG α-subunit or only on the N-glycans of hCGβ will not be recognized by antibody B152. Apparently, measurements of these core 2 hCG isoforms have important clinical application in early pregnancy during which they are the predominant isoform of hCG until the 6th week of gestation. The secretory pattern of these isoforms can be used to predict the health status of the pregnancy in fertility clinics. Moreover, the measurements of these core 2 hCG isoforms are more useful than standard hCG for the prediction of Down syndrome pregnancies. The core 2 isoforms are also of important use in cancer diagnosis and monitoring since their concentration appears to correlate with malignancy.
