Urogenital symptoms in women with Tarlov cysts

Abstract: Patients with symptomatic sacral Tarlov cysts frequently report pain and neuropathy related to the lower back, pelvis and urogenital system. As compared to the general population, urinary urgency and urodynamic findings associated with urgency were more frequent in our patient sample. These findings suggest that Tarlov cysts may have a clinically significant impact on urogenital function.

“… Note : The Urinary Symptom Profile is a standardized validated questionnaire of bladder symptoms addressing stress urinary incontinence (maximum score 9), overactive bladder (maximum score 21) and low stream (maximum score 9) symptoms [12]. The Constipation Scoring System is an eight‐item questionnaire of bowel symptoms that has been validated in the general population to assess constipation (maximum score 30) [13].…”

“…The sacral splanchnic innervation within the sacral nerve roots may also get affected, causing urogenital and bowel dysfunction that is frequently reported in cohorts involving patients with presumed symptomatic TCs [9][10][11]. A large prevalence of overactive bladder (OAB; including urgency with or without incontinence, frequency and nocturia) has been reported in a dedicated study [12], whereas other studies mentioned a high rate of urinary disorders, pooling together storage (e.g., symptoms of OAB or stress incontinence) and voiding symptoms (hesitancy, low stream, urinary retention) [10,11]. However, the extent to which these symptoms can be attributed to neurological injury remains unclear in the absence of objective means of demonstrating sacral nerve root injury.…”

“…In diesem Zusammenhang wurde sogar eine Sakrumfraktur beschrieben [12]. Zudem wurden Serien und Fallberichte publiziert, in denen abdominelle und urogenitale Symptome beschrieben werden [13,14]. Die Beschwerden können in aufrechter Körperposition oder während eines Valsalva-Manövers akzentuiert sein.…”

“…Rare cases of chest, abdominal, or pelvic pain have also been reported. [9][10][11][12][13][14][15] The gold standard in the diagnosis of perineural cysts is magnetic resonance imaging (MRI), which allows the assessment of the relation between the cyst, the spinal nerve root, and the neighboring structures.…”
