Open TVP still remains a valid surgical option in contemporary environment where advanced techniques for transurethral resection of the prostate and laparoscopic prostatectomy are unavailable. Most complications occur in the early postoperative period, with bleeding requiring several units of blood transfusion accounting for the commonest complication. This should be explained to patients during the preoperative counselling.

Cancer incidence in Africa is increasing as a result of aging populations, lifestyle changes, and improved detection. However, cancer etiology, prevention, and control in Africa are poorly understood, and survival rates from cancer are among the lowest in the world. Poor cancer survival suggests that improvements in health care, patient education, and relevant research are critically needed. An increase in research infrastructure is required to achieve adequate screening, diagnosis, and treatment of cancer in Africa. While research is a basis for clinical practice and policy changes in resourceabundant areas, the research infrastructure in Africa is deficient in many areas. Building infrastructure for research includes creating supportive environments for research endeavors, securing funding for resources and research personnel, developing training and mentorship opportunities, and building collaborative research with other institutions. Goals for the development of cancer research in Africa may include research on cancer incidence and mortality, cancer etiology and risk factors, clinical research with a focus on early diagnosis/treatment and palliative care, health economics, and community-based participatory research.

There are significant problems surrounding lack of access to surgical services, surgical training and surgical safety in Africa. There are many reasons for this. A recent report suggests that the ongoing healthcare workforce crisis is set to get worse in sub-Saharan Africa, with an estimated shortfall of 800,000 health professionals by 2015 and a required additional wage bill of approximately US $2.6 billion. Reasons include a lack of medical school places to meet demand; poor wages, facilities and infrastructure; impact of the HIV/AIDS epidemic; and migration to urban areas and developed countries. For example, although Kenya has trained 300 surgeons since 1972 only 120 of them remain in public service, with 27 of its 63 district hospitals having no qualified surgeons.
