Urothelial Carcinoma With Villoglandular Differentiation

Abstract: Urothelial neoplasms are the commonest neoplasms of the urinary bladder. Many variants of urothelial neoplasms have been described in the literature with diagnostic, therapeutic, and prognostic significance. We describe a rare case of urothelial neoplasm with villoglandular differentiation along with its immunohistochemical profile arising in an elderly male. Its poor prognosis signifies its need to be recognized.

“…Membranous positivity was also further subcategorized as complete or incomplete. The clinicopathological features of one of the cases had been reported earlier (3). All the samples were TURBT (Transurethral resection of bladder tumour) specimens and the radical cystectomy specimen of one of the cases (previously reported case) was also obtained.…”

“…Histomorphologically, this entity shows a divergent differentiation towards gastrointestinal epithelium resembling an intestinal adenomatous polyp coexisting with and intricately admixed with urothelial carcinoma. Most of the cases of UCVGD are reported to be associated with a high-grade component of urothelial carcinoma and are found to show lamina propria and/or detrusor muscle invasion, implying a poorer prognosis (1,(3)(4)(5). Villous adenoma is also a rare preneoplastic condition in the genitourinary tract and presumably arises from the cloacogenic remnant.…”

“…Till date, histopathology of only a few cases have been described in the literature. [1][2][3][4] To the best of our knowledge, the cytological features that could allow the diagnosis of this entity have not been described.…”

“…1 This entity has recently been described in a case series and two case reports. [2][3][4] Histopathology shows an admixture of papillae with finger like filiform processes, the papillae being lined by the transitional lining epithelium whereas the filiform villi are lined by columnar epithelium simulating colonic epithelium. In addition, the surface also shows cribriform and glandular architecture.…”
