Tooth fusion is a dental anomaly with a prevalence of 0.1% in permanent dentition. The aim of this paper is to present the occurrence, region of fusion, and prognosis of fused tooth which is a very rare case in the literature. The authors present a very rare case with a maxillary right lateral incisor fused to a supernumerary tooth on the palatal side. The eruption was delayed, but eruption occurred naturally at 9 years and 6 months. However, cone-beam computed tomography at 11 years and 6 months revealed it was diagnosed as a fused tooth. The authors undertook a literature review over the past 10 years and found 30 articles about fused teeth between permanent teeth and supernumerary teeth. It was more common in males than females and there were no differences depending on the occurrence. Mesiodistal fusion was more common in the incisor region, while labiopalatal fusion was more common in the molar region. Most cases required dental treatment with a surgical approach, root canal treatment, or orthodontic treatment. The present study highlights the importance of long-term follow-up and suggests the region of fusion and prognosis of the fused tooth may vary depending on the occurrence.

BACKGROUND Fused teeth usually involve several complications, such as the development of caries in the groove between fused crowns, tooth impaction, diastemas, aesthetic and periodontal problems, and pulpal pathosis, due to the complex anatomical structure of fused teeth. A thorough diagnosis is paramount to forming an accurate treatment plan and obtaining a favourable prognosis. With the advent of cone-beam computed tomography (CBCT), accurate 3-dimensional images of teeth and their surrounding dentoalveolar structures can now be readily obtained, and the technology can accurately provide a minimally invasive approach to acquire detailed diagnostic information. Therefore, we utilize CBCT data herein to generate a digital model for the infected region in a patient, and this model enables us to better plan the management of his case. CASE SUMMARY This report details the diagnosis and endodontic treatment of a rare case involving a fused maxillary second molar and two paramolars with apical periodontitis. The patient experienced pain upon biting and cold sensitivity in the area of the maxillary left molar. No caries or other defects were identified in these teeth, and a normal response to a pulp electric viability test was observed. With the aid of CBCT and digital model technology, we initially suspected that the infection originated from the isthmus between the maxillary second molar and two paramolars. Therefore, we only treated the isthmus by an endodontic approach and did not destroy the original tooth structure; furthermore, the vital pulp was retained, and good treatment outcomes were observed at the 24-month follow-up. CONCLUSION This finding may provide new insights and perspectives on the diagnosis and treatment of fused teeth.

RESUMEN:Actualmente los factores que influyen en la supervivencia de los dientes trasplantados han podido ser controlados con el uso de la tecnología. El autotrasplante dental guiado ha logrado más predictibilidad y eficiencia, además, ha reducido los tiempos de transferencia desde la extracción hasta el trasplante. El objetivo de esta revisión es describir los protocolos de autotrasplante dental guiado, sus tasas de supervivencia y éxito publicados en la literatura actual. Esta revisión fue realizada siguiendo la pauta PRISMA. La búsqueda se realizó en MEDLINE, Google Académico, ScienceDirect y SciELO, con los términos "autotransplant", "autotransplantation", "autotransplanting", "dental", "tooth", "teeth", "guided", "guide" con filtro de publicación de 10 años. Se realizó evaluación de riesgo de sesgo mediante pautas Joanna Briggs Institute (JBI) a los estudios, incluyendo en esta revisión sólo con riesgo medio y bajo. Los datos de cada artículo se tabularon en una tabla realizada en el procesador de texto en línea "Google Docs". Diez estudios cumplieron los criterios mencionados y fueron incluidos. Fueron evaluados un total de 37 pacientes entre 9 a 64 años. Los dientes donantes más frecuentes fueron premolares y terceros molares. En la mayoría de los casos los pacientes se encontraban sin antecedentes mórbidos de relevancia. El éxito fue evaluado mediante diversos criterios clínicos y radiográficos. Por otro lado, la supervivencia fue evaluada durante los seguimientos respecto a la permanencia del diente en boca. Este tipo de tratamiento no es muy conocido y los estudios incluidos fueron escasos, por otro lado, estos son de bajo nivel de evidencia (reportes de casos y serie de casos). Los protocolos evaluados difieren en algunas características, sin embargo, todos logran altas tasas de supervivencia y éxito. Igualmente, se presentan algunos fracasos, dónde los dientes debieron ser extraídos por movilidad e inflamación.PALABRAS CLAVE: autotrasplante dental guiado, tomografía computarizada de haz cónico, impresión 3D.
