Use of Antithrombin Concentrate in Stable Diffuse Intravascular Coagulation

Abstract: A case report is presented of a 39-year-old woman suffering from a myelodysplastic syndrome, who developed a moderately severe stable diffuse intravascular coagulation (DIC) during an uneventful pregnancy. Intravenous administration of antithrombin III concentrate was attempted to combat the DIC. Improved survival of platelet transfusions, facilitating supportive care, suggested some therapeutic benefit. In contrast, only minimal effects on the results of the coagulation tests were observed.

“…47 Since antithrombin seems to be the most im portant inhibitor of the enzymes generated during clotting, it was logical to study the effect of anti thrombin concentrates in patients with decreased levels, either congenital or acquired. Generally, positive results were reported in patients with DIC of various causes, 4,[8][9][10]17,31,36,69,84 although in one report antithrombin had no effect in patients sub jected to LeVeen shunts with subsequent DIC. 16 None of the patients was, of course, treated with antithrombin alone, and in some instances it was the therapy of last resort.…”

“…72 Patients with DIC of various causes have also been treated with human antithrombin concentrates. 4,8,9,31,36,69,84 The response was generally favorable, although most patients were treated with other therapeutic modalities as well.…”
