Use of dienogest over 53 weeks for the treatment of endometriosis

Abstract: Long term DNG treatment beyond one year for endometriosis proved to be effective and safe. Ovarian chocolate cysts were markedly reduced by short-term use of DNG, while a longer duration was required to reduce the size of adenomyosis. The decision regarding the choice of therapy lies with the individual clinician, considering a balance of efficacy with expense and adverse effects.

“…Our data are in agreement with previous reports [812] in that dienogest was effective in reducing endometriosis-associated pain. Specifically, it is noteworthy that the size of recurrent endometriomas was significantly reduced by dienogest treatment, as in a previous Japanese study [12]. Although the mechanism is not clear, dienogest treatment might suppress extracellular signal-regulated kinase 1/2 pathways, inhibit mammalian target of rapamycin, induce autophagy, and promote the apoptosis of endometriotic cells [13].…”

“…Additionally, dienogest was seen to prevent endometriosis‐related pain relapse and the recurrence of endometriosis lesions after surgery . In fact, dienogest has been deemed safe to use in the long term to relieve dysmenorrhea without adverse effects due to hypoestrogenism, such as decreased bone mass and hot flushes . However, we have seen that several patients with uterine adenomyosis‐related dysmenorrhea were resistant to treatment with dienogest.…”
