Use of epsilon aminocaproic acid (EACA) in the preoperative management of ruptured intracranial aneurysms

Abstract: The authors report their experience with the use of epsilon aminocaproic acid (EACA) in the preoperative management of a series of patients with ruptured intracranial aneurysms. A similar series of patients was taken as control. They found that EACA is of definite value in preventing recurrent hemorrhage in the preoperative period. The significance of antifibrinolytic therapy in ruptured intracranial aneurysms is discussed.

“…In this situation, use of antifibrinolytic agents, ex pecting to prevent recurrent hemorrhage at least in a few postictal weeks, were widely accepted and there appeared many reports supporting its favorable results (5,15,18,19,22). Also possible complications have been extensively investigated.…”

“…4 None of the previous studies was controlled or doubleblind. Sengupta et al 10 reported good results with antifibrinolytic treatment on the re-bleeding rate in a controlled trial but they did not report the effect of the treatment on mortality and their control group comprised more severely ill patients than their active treatment group. In their active treatment group only 8 of 66 patients were not operated on, while in the control group, in 31 of 76 patients, there was reason to refrain from surgery making their groups non-comparable.…”

“…However, given after a subarachnoid hemorrhage, EACA has been reported to cause middle cerebral artery thrombosis. 17 Our patient was obese, a state associated with hyperestrogenism, 18 which is a known risk factor for hypercoagulability. Theoretically, this could have made her more susceptible to the development of cerebral sinus thrombosis during EACA administration.…”
