Use of left ventricle assist device in a pregnant woman with acute aortic and coronary dissections

Abstract: We report the case of a 31-year-old female in her 32nd week of pregnancy, who experienced an acute type A aortic dissection resulting in severe aortic insufficiency, dissection of the left anterior descending coronary artery, and left ventricular infarct with ejection fraction of 20%. After a successful cesarean section, she underwent a Bentall operation and three-vessel coronary artery bypass grafting. Despite maximal inotropic support, her cardiovascular indices deteriorated postoperatively. On the first pos… Show more

“…The literature retrieval yielded 96 reports, comprising 85 case reports and 11 case series or original articles, with 122 patients . Mean patient age (n = 120) was 31.6 ± 4.7 years (range, 19–43 years; median, 31.5 years) on admission.…”

“…In some patients with significant ischemic myocardial dysfunction, successful use of mechanical circulatory support as a bridge to recovery has been reported. 11,12 Location of the intimal tear in the aortic arch or more distally was associated with reduced hospital survival. Lansman and colleagues 13 observed reduced short-and long-term survival in these patients, and found that they tended to be older, less likely to be male or present with aortic regurgitation, and more likely to present with rupture.…”

“…Asakura et al report their experience in which 3 out of 9 patients survived using left sided mechanical support following repair of acute type A aortic dissection [1]. Kolesar et al similarly report the use of a left ventricular assist device in a pregnant woman with acute aortic dissection [4]. There is only one report of the successful utilisation of an RVAD in RV failure following an aortic dissection repair.…”
