Use of monoclonal antibodies in diagnosis of paracoccidioidomycosis: new strategies for detection of circulating antigens

Abstract: The precise diagnosis of paracoccidioidomycosis, in most cases, is established by direct methods and indirect immunological tests. The latter method is reliant on the identification of the host's humoral responses, which are usually impaired or absent in patients with severe juvenile forms of the disease and in immunocompromised patients. Determining disease activity or assessing treatment responses by measuring antibody levels is difficult, since antibody titer may remain elevated or persist at stationary lev… Show more

“…Smear or culture can also be diagnostically useful, but P. brasiliensis grows extremely slowly. Serologic examination by immunodiffusion (Bueno et al, 1997;Gomez et al, 1997) or particularly the complement fixation test is useful mainly in epidemiological studies.…”

“…Finally, the definition of cure in PCM remains a matter of debate, and more prospective studies are needed. Other laboratory parameters, such as antigenaemia in urine or serum [16,58,59], may help to define the time points to change or stop therapy more accurately.…”

“…Both antigens also could be detected in the CSF and BAL (58, 59) and in urine (105). Detection of a gp87‐kDa antigen also has been reported but would appear to be less useful because of cross‐reactions with other fungi and mycobacteria (106). However, whether these methods are available for routine clinical use is unknown.…”
