Use of Rapid Drug Desensitization in Delayed Hypersensitivity Reactions to Chemotherapy and Monoclonal Antibodies

Abstract: Background:Rapid drug desensitization (RDD) allows first-line therapies in patients with immediate drug hypersensitivity reactions (DHR) to chemotherapeutic drugs (ChD) and monoclonal antibodies (mAb). Desensitization in delayed drug reactions has traditionally used slow protocols extending up to several weeks; RDD protocols have been scarcely reported.Patients and Method:We retrospectively analyzed the patients referred to the Allergy Department, who had experienced a delayed DHR (> 6 h) related to a C… Show more

“…Moreover, patients with delayed hypersensitivity reactions can undergo RDD but reports of its efficacy are scarce. Eight patients referred to an Allergy Department with a delayed hypersensitivity reaction to chemotherapy performed RDD using a protocol with 8–13 steps, with temozolomide, bendamustine, gemcitabine, and docetaxel, within 4.6–6.5 h, with 13 breakthrough mild reactions reported but even though all patients completed their treatment [17 ▪▪ ].…”

“…When no alternative agent is available and the use of the culprit drug is mandatory, drug desensitization becomes the sole choice for management [5]. However, there is little experience with delayed DHRs involving chemotherapeutic agents [6].…”

“…The allergology study is challenging because of the short time available to perform the skin tests, concomitant corticosteroid treatment, and the lack of validation of skin tests for DHRs to the drug [6]. The lymphocyte transformation test can be useful as an in vitro diagnostic tool [1] for demonstrating the underlying mechanism.…”
