BackgroundA number of studies have shown the feasibility of using adjunctive drugs in late onset psychosis (LOP).AimTesting hypothesis that among LOP people treated with antipsychotics and antidepressants, basing on certain clinical characteristics a subgroup of patients might be distinguished, for whom adjunctive therapy is advantageous. This subgroup might be identified by measurement of blood biochemical parameters.Methods59 in-patients with LOP, treated neuroleptics and antidepressants, were included, and followed in real clinical practice. Database containing demographic, clinical data (scores by PANSS, CDSS, CGI-S, HAMD-17), prescribed therapy, adverse effects of antipsychotic and antidepressant treatment, and blood biochemical parameters (enzymatic activities of glutamate- and glutathione metabolism enzymes in platelets and erythrocytes) at baseline and after the treatment course was created.ResultsThree groups of patients (Gr1, Gr2, and Gr3), based on the adjunctive therapy usage were identified: Gr1 (n = 16) was without adjunctive therapy, two other groups (Gr2 and Gr3) were with adjunctive medicines, such as 2-ethyl-6-methyl-3-hydroxypyridine succinate (EMHS; Gr2, n = 20), or other drugs, such as citicoline, cerebrolysin, cortexin, actovegin, gliatilin (choline alfoscerate; Gr3, n = 23). The enzymatic activities were assessed also in the matched control group (n = 38). In all three patient groups, as compared with controls, activity of erythrocyte glutathione reductase was decreased at baseline and after the treatment course. In Gr2, unlike Gr1 or Gr3, there was a significant decrease in baseline glutamate dehydrogenase and glutathione-S-transferase activities. Certain clinical criteria were also elucidated for prescription of EMHS as adjunctive therapy for patients of Gr2. Glutamate dehydrogenase and glutathione-S-transferase activities returned closer to control levels after the treatment course in Gr2, unlike Gr1, where they declined yet more after psychotropic treatment without adjunctive medicine. Different significant links between biochemical parameters and scores by clinical scales were observed in Gr1, Gr2, and Gr3, some having predictive value for evaluation of antipsychotic treatment efficacy.ConclusionWe demonstrate the validity of adjunctive neuroprotective medicines’ usage in addition to antipsychotic and antidepressant therapy in distinct subgroups of patients suffering with LOP, especially those who have prominent side effects accompanying their psychotropic treatment. Returning of biochemical parameters to control range following the treatment course observed in patients of the subgroup treated with adjunctive EMHS is evidence for their metabolism normalization.

Remaxol was evaluated as a means of hepatotropic support for patients with COVID-19 receiving antiviral therapy. Patients were observed in two groups of 30 people each and received intravenous infusions of Remaxol (400.0 ml) or “active placebo” (400.0 ml. 0.9% sodium chloride) for 10 days. In these groups of patients, an analysis was made of the dynamics of clinical parameters and laboratory tests that characterize the functionality of the liver. The results of the analysis made it possible to state that the use of the corrector of cellular and energy metabolism metabolism Remaxol as a hepatoprotector in complex therapy contributes to favorable clinical dynamics, stabilization of biochemical parameters of liver function and allows completing the course of etiotropic therapy in all patients.Good tolerability of the drug “Remaxol” and the absence of pronounced side eﬀects were noted. The course of Remaxol made it possible to carry out therapy with antiviral agents without dose adjustment and duration of treatment.

Резюме: Значительная часть клеток Догеля 2-типа в кишечнике млекопитающих имеют очень короткие нейроматозные выросты цитоплазмы -парафиты, тесно прилегающие к стенке кровеносных и лимфатических микрососудов. Они могут быть случайными или истинными афферентными синапсами этих нейронов, участвующимив иннервации сосудов кишечника. Цель работы -на морфологическом уровне доказать участие парафитов клеток Догеля 2-го типа в иннервации микрососудов кишечника. Изучены импрегнированные серебром тотальные препараты стенки тонкой кишки интактных беспородных кошек (n=5). Установлено, что нейроциты, расположенные на периферии ганглиев, имеют короткие неветвящиеся отростки и образуют локальные контакты со стенкой микрососудов. На микроскопическом уровне эти контакты выявляются как четко сформированные рецепторные структуры в виде претерминальных или терминальных бляшек. Тонкие неветвящиеся отростки отходят не только от области перикариона нейронов, но и от парафитов. Удалось также обнаружить синцитиальные связи этих отростков с перикарионами рядом расположенных нейронов. Часто парафиты, имеющие большие по площади ламеллы, были связаны с перикарионами узкими цитоплазматическими перемычками. Это дает основания считать, что подобные ламеллы являются потенциальными цитомерами в зоне микрососудистого русла кишечника. Парафиты клеток Догеля 2-го типа участвуют в иннервации микрососудов кишечника и являются потенциальными цитомерами, способствующими регенерации клеточных элементов стенки микрососудов.Ключевые слова: клетки Догеля 2-типа, сосуды кишечника, иннервация кишечника Summary: A significant part of the II nd type of Dogiel cells in the intestines of mammals have very short neuromatous processes of the cytoplasm -paraphytes, closely adjacent to the wall of blood and lymphatic microvasculature. They can be random or true afferent synapses of these neurons involved in the innervation of the intestinal vessels. The purpose of the work is to prove at the morphological level the participation of paraphytes of the II nd type of Dogiel cells in the innervation of intestinal micro vessels. Total, impregnated by silver preparations of the wall of the small intestine of intact outbred cats (n=5) were studied. It was found that neurocytes located on the periphery of the ganglia have short, unbranched processes and form local contacts with the wall of micro vessels. At the microscopic level, these contacts are detected as clearly formed receptor structures in the form of preterminal or terminal plaques. Thin non-branching processes depart not only from the area of the pericarion of neurons, but also from paraphytes. It was also possible to detect the syncytial connections of these processes with the pericarions of adjacent neurons. Often, paraphytes with large lamellae were contacted with narrow cytoplasmic ligaments with pericarions. This suggests that such lamellae are potential cytomers in the area of the microvascular channel of the intestine.
