Use of Surgical Laser for Excision of a Neurofibroma Associated With Neurofibromatosis Type-1

Abstract: Introduction: Neurofibromatosis type 1 (NF1) is an autosomal dominant genetic disorder that affects approximately 1/3500 individuals. Various bone manifestations and peripheral nerves neoplastic lesions associated with NF1 are seen in the jaws. Several oral manifestations may occur in this disorder; therefore the dentist's knowledge and multidisciplinary management of these patients are extremely important. Case Presentation: In the present article, we present the use of a high-power surgical laser to excise a… Show more

“…Management of neurofibromas has traditionally involved simple scalpel excision, and more recently incisional biopsy followed by laser ablation or excisional laser biopsy. 12 Other recommendations have included removal of the underlying periosteum and inclusion of a 5 mm surgical margin. 11,13 For suspected gingival neurofibromas, clinicians should carefully balance the decision to achieve an overly conservative excision, risking formation of a mucogingival defect and potential need for gingival grafting, versus the incorporation of somewhat wider tissue margins to lessen the chance of lesional recurrence.…”

“…Moreover, NF1 patients present a higher lifetime cancer risk (59.6%) in contrast to the general population (30.8%) [ 22 ]. Malignant peripheral nerve sheath tumor (MPNST) is the main malignancy which develops in 5% to 13% of NF1 patients [ 4 , 18 , 22 ], with poor prognosis [ 23 ]. Intra-cranial malignancies such as gliomas, ependymoma, and childhood astrocytomas are also well known [ 12 , 23 ].…”

“…Intra-osseus jaw bone lesions related to NF1 are also reported and consist mostly of the widening of the mandibular canal which may be associated with an irregular border of the canal and enlarged mandibular foramina unrelated to any tumor mass [ 3 , 4 , 7 , 9 , 16 , 18 ]. Intra-osseus neurofibromas resulting in well-defined radiolucent lesions have also been reported [ 7 , 9 ].…”
