Use of the Omentum Flap as Additional Soft-Tissue Cover for Abdominal Wall Defects Reconstructed with Gore-Tex

Abstract: The omentum flap served as an additional soft-tissue cover over the Gore-Tex repair to prevent exposure in the event of infection or flap breakdown. This technique is useful in situations in which delayed wound healing is anticipated or when large quantities of prosthetic material are used.

“…This has been described with a muscle splitting dissection lateral to the semilunar line, bringing out the omental pedicle in an oblique direction. 12 Utilizing purely autologous tissues, as we did, avoids most of the complications of the other techniques. It is readily available, is relatively cost-effective, especially advantageous in the developing country, and is less technically demanding.…”

“…The presence of the liver on the right side may have prevented the herniation. 12 Tunneling the omental flap subcutaneously to the recipient site through an adjacent but separate opening through muscle may also have prevented the herniation. This has been described with a muscle splitting dissection lateral to the semilunar line, bringing out the omental pedicle in an oblique direction.…”

“…The pedicled omentum is an autogenic tissue flap with plentiful blood vessels and connective and lymphatic tissues (42). It has been a special option to reconstruct the abdominal wall, because of its well-off blood supply and immunologic properties (43). By using the omental flap as an interpositional flap, we harness these properties to facilitate wound healing and to promote remodeling of HADM after implantation.…”

“…[5,6] Adsorbable mesh can be used in these situations; however, this solution is temporary and predisposes the patient to multiple operations and a staged abdominal wall reconstruction to achieve an acceptable functional result (sometimes, the very high operative risk precludes a two-stage surgical procedure [7] ). A number of techniques such as component separation, [8] musculocutaneous [9,10] or pedicled omentum [11][12][13] flaps have been suggested as a good alternative for autologous tissue repair, but the size of the defect often limit such possibilities, and the potential risk for donor site morbidity should be taken into account. [14] Recently, the development of biologic meshes has shown successful rates in the management of these parietal wall defects.…”
