A 3‐year‐old llama was presented for management of bilateral ureter obstruction. A general anaesthetic was performed to allow a midline coeliotomy to be undertaken. Due to current legislation, the provision of adequate analgesia can be challenging. The transverse abdominis plane (TAP) block is a locoregional technique that can be used to provide analgesia of the ventral abdominal wall in numerous species. The TAP block was performed in this case to provide intraoperative analgesia using mepivacaine. This is the first time this technique has been described in the llama. Based on intraoperative monitoring, the TAP block appears to provide analgesia intra‐operatively. Due to the degree of damage to the ureters, the llama was euthanised under general anaesthesia, so an assessment of postoperative analgesia provided by the block could not be assessed.

Transversus abdominis plane block in rats: Preliminary cadaveric studies

The transversus abdominis plane (TAP) block is an ultrasound guided regional anaesthetic technique used to provide analgesia to the abdominal wall. Research in humans and cats has demonstrated that TAP blocks reduce pain and post-operative opioid requirements after abdominal surgery. To date TAP blocks have not been described in rats. The optimal technique to employ when performing TAP blocks is controversial with single point injection techniques failing to reliably provide adequate coverage of the cranial abdominal wall. It has been suggested that performing a two point injection may provide more reliable coverage of the cranial abdominal wall. The objective of this study was to determine the feasibility of performing ultrasound guided TAP blocks in rat cadavers and to evaluate whether performing a two point technique provides greater spread of injectate than a one point technique when administering the same total volume of methylene blue solution. Twenty-three, four-month-old, female Sprague Dawley rat cadavers weighing 506 ± 78 g were used. TAP blocks were performed using a total of 1 ml/kg of methylene blue solution. Overall success rates for injections were 21.7% (13.6–32.8%). Single point injection area of spread was 87.8 ± 32 mm2 compared with 102.4 ± 17 mm2 for the two point injection technique. Due to the low success rate the use of TAP blocks using the current technique cannot be recommended. Two point injection technique appears to provide greater spread; however, additional data is required to draw meaningful conclusions.

Open Approach to the Transversus Abdominis Plane in Horses: A Cadaver Feasibility Study

The study’s objective was to evaluate the feasibility and dispersion of an open approach to the transversus abdominis plane (TAP) block in eight adult equine cadavers. A ventral midline incision was made, starting 2 cm cranial to the umbilicus and extending 25 cm cranially. In total, 0.5 mL/kg of new methylene blue (NMB) was injected per horse, divided into six injections. Using an 18 g, 8 cm Tuohy needle, three injections were made per side. The needle was guided blindly into the TAP using palpation. A 60 mL syringe was attached directly to the needle, depositing ~0.08 mL/kg at each site. The time to complete the injections was recorded for each cadaver. Following injection, the ventral body wall was dissected to determine if the dye was present within the TAP space as well as to measure the extent of the dispersion of the dye, the cranial to caudal extent, and the width of the dye’s spread. Complete deposition of NMB into the TAP (six of six sites) was achieved in 5/8 horses. The median time needed to perform all the injections was 263 s. Increased adiposity (retroperitoneal fat) was associated with unsuccessful injections. This approach to the TAP was easily and quickly performed, though less successful in horses with increased retroperitoneal fat and increased BCS.
