Use of venous couplers in microsurgical lower extremity reconstruction: A systematic review and meta‐analysis

Abstract: Background Free tissue transfer for lower limb reconstruction has become a workhorse procedure for limb‐salvage. Compared with other recipient sites, the lower extremity has a higher risk of microvascular complications, in particular with venous anastomosis. The study's objective is to evaluate the evidence, safety, and efficacy of venous coupler use in microsurgical anastomosis in lower limb reconstruction to provide objective appraisal of the surgical techniques. Methods A systematic review (SR) and meta‐ana… Show more

“…For maximum comparability and data granularity, we chose to analyze complete SF-36 data only, instead of including Short Form 12 as well, which appears to be a more prevalent measure in trauma outcome research than in burns and sepsis. Notwithstanding all aforementioned limitations, MAs performed on case series can provide strong evidence of potential cause-effect relationships, and clinicians can learn from them irrespective of the limitations 48–50 …”

“…As is seen with other reviews and meta-analyses, our study also carries limitations. First, as stated in prior meta-analyses, the overall quality of a meta-analysis is limited by the quality of studies it includes 49,50 . The majority of the included studies were retrospective nonrandomized case control studies, which suffer from inherent bias, including selection bias.…”

“…First, as stated in prior metaanalyses, the overall quality of a meta-analysis is limited by the quality of studies it includes. 49,50 The majority of the included studies were retrospective nonrandomized case control studies, which suffer from inherent bias, including selection bias. The inconsistency in the included studies highlights that well-designed prospective cohorts are needed.…”

“…Recently, Haug et al and Maruccia et al provided systematic reviews about venous couplers(Haug et al, 2020 (Online ahead of print);Maruccia et al, 2020). Haug et al reported comparable outcomes of venous couplers to those of hand-sewn anastomosis in lower limb reconstructions, specifically finding no difference in venous compromise(Haug et al, 2020 (Online ahead of print)). However, sample size and quality of data was limited.…”
