Use of Voriconazole in Treatment of Scedosporium apiospermum Infection: Case Report

Abstract: We report a case of disseminated Scedosporium apiospermum infection in a neutropenic patient with acute myeloid leukemia. Due to progression of the mycosis after 7 days of amphotericin B lipid complex therapy and to high susceptibility of the mold to voriconazole in vitro, the patient was treated with intravenous voriconazole. After a few days of therapy, fever disappeared and skin lesions improved. However, the patient died after 1 month due to intestinal bleeding. Despite a negative outcome, this case seems … Show more

“…Voriconazole is a new triazole with good in-vitro activity against a range of moulds, including S. apiospermum [60]. The good in-vitro activity and satisfactory experience in patients after antifungal drug therapy mean that voriconazole is currently the drug of choice for this infection [91,115,[128][129][130].…”

“…In vitro data suggest that S. apiospermum isolates can be susceptible to voriconazole, itraconazole, and miconazole [10][11][12][13]. Previously, voriconazole was used successfully in the treatment of S. apiospermum infection [2,14,15].…”

“…42,43 Voriconazole has been used with therapeutic success in both disseminated and central nervous system scedosporiosis. 32,[44][45][46] The use of intravenous voriconazole in the present case was problematic given the patient's renal failure. Our patient had only a few doses of voriconazole prior to her death; it is unlikely that any antifungal agent would have been effective given the extent of her infection.…”
