Usefulness of ambulatory blood pressure monitoring in pregnant women with type 1 diabetes.

Abstract: Our study confirms the early increase of BP in patients who will develop PIH and suggests that nighttime systolic BP >105 mmHg in the second trimester is a useful predictor of PIH. ABPM may be useful in screening for PIH in pregnant diabetic women.

“…The diurnal BP is of value in normo‐ as well as microalbuminuric women where first and second trimester diurnal BP tend to be significantly higher in those women who developed preeclampsia (2, 3). Blood pressure did not affect intrauterine growth in our series, as birth weight ratio in women with preeclampsia was similar to normotensive achieved by timing the delivery early.…”

“…Several studies on blood pressure during pregnancy have shown a decrease in first and second trimester (1–3). The fall coincides with increasing cardiac output and blood volume and is explained by lowering of the vascular resistance.…”

“…The role of ABPM in this setting is not well established, however. On one side, some studies suggest its possible usefulness in monitoring pregnant women and in the early identification of pregnancy-induced hypertension, especially when nocturnal BP is considered (40). On the other side, according to some authors, ABPM adds little information to that already provided by microalbuminuria assessment as far as prediction of preeclampsia is considered (41).…”

“…Evaluation of a series of blood pressure (BP) measurements, such as diurnal ambulatory measurements, includes several sources of variation or error, due to the apparatus itself, the circumstances of measurements, and the individual's characteristics. In qualifying for certain standards of performance, the apparatus is evaluated in certain well‐defined clinical settings, and the ability of prediction is graded to correspond to a clinical outcome (1–5). However, rarely the reading of a BP measurement is evaluated against the presumed true value of BP.…”

“…However, rarely the reading of a BP measurement is evaluated against the presumed true value of BP. Several studies have aimed at identifying those women at risk of hypertensive disorders in pregnancy, whereas a relative few have evaluated the measurement per se during pregnancy (1–3). These studies have identified various times and settings during pregnancy when measuring is worthwhile (3).…”
