Background Clinical characteristics of the lesions are used to identify activity and damage in localized scleroderma (LoS). For high‐frequency ultrasound (HF‐US), the features of active lesions were described. Materials and Methods Clinical signs of activity and damage in LoS lesions were assessed with the use of Localized Scleroderma Cutaneous Assessment Tool (LoSCAT) and HF‐US by two examiners independently. All US images were obtained using a 20 MHz HF‐US (DermaLab System, Cortex Technology, Hadsund, Denmark). The dermal thickness (DT) and echogenicity (intensity score, IS) of the LoS lesional dermis were measured in the area of each lesion with the highest score for erythema (ER), skin thickness (ST), and dermal atrophy (DAT). Measurements were compared to the site‐matched unaffected skin. The relative difference of DT and IS values was calculated between each lesion and its normal control for comparison among different clinical scores for ER, ST, and DAT. Results A total of 92 lesions in 40 adult patients were examined with HF‐US. Thirty one lesions were erythematous, 26 were in sclerosis, and 35 were in atrophy. A correlation between the clinical evaluation of the LoS lesions and US measurements was found. The sensitivity and specificity of HF‐US were 97% and 90%, respectively. The positive predictive value was 83%, negative predictive value—98%. Interrater reliability was excellent for LoSCAT and HF‐US findings. Conclusion High‐frequency ultrasound allows an accurate assessment of the inflammatory and fibrotic skin lesions in LoS.

ÖSSZEFOGLALÁSAz elmúlt évtizedekben az orvosi képalkotás fejlődése jelentős előrelépéseket hozott a bőrgyógyászatban. Bár jelenleg a hisztopatológiai vizsgálat az "arany standard" diagnosztikus módszer, a már-már szövettani felbontást lehetővé tévő in vivo, non-invazív képalkotási módszerek számos bőrgyógyászati kórkép vizsgálatára hatékonyan alkalmazhatók. A szerzők összefoglalják a bőrgyógyászat területén már széleskörűen alkalmazott, valamint az ígéretes, de egyelőre kísérleti stádiumban lévő bőrgyógyászati képalkotó módszereket. Az egyes modalitások elméleti hátterének ismertetése mellett részletezésre kerülnek a különböző technikák klinikai alkalmazásának lehetőségei. However, histopathological evaluation still remains the gold standard diagnostic method. Novel, in vivo non-invasive imaging modalities can provide near histological resolution and can be applied in the assessment of various dermatological diseases. In this paper, the authors review the clinically applied imaging techniques among the newest experimental devices. After the description of the basic principles and imaging properties of these techniques, the authors also discuss their clinical applications.

Objectives-Basal cell carcinoma (BCC) is the most common dermatologic malignant skin cancer. Infiltrative histologic variants are more aggressive and require wider surgical margins or Mohs surgery, in contrast with noninfiltrative variants, which are commonly treated with standard surgical excision. Elastography has not been used to date to differentiate between the histologic variants of BCC. The purpose of this study was to differentiate infiltrative from noninfiltrative BCCs on elastography.Methods-A total of 31 facial BCCs were studied. Preoperatively, color Doppler ultrasound and strain elastographic examinations of the lesions were performed. The size, intralesional vascularization, and presence of hyperechoic dots were considered relevant B-mode and color Doppler variables. Strain ratios of the tumors were obtained with respect to adjacent healthy tissue. Increased marginal stiffness, which was considered hardness in greater than 50% of the tumor margin, was also blindly evaluated. Histologic confirmation and subtyping (infiltrative or noninfiltrative) were performed in all cases.Results-Infiltrative BCCs did not differ in the size, presence or absence of hyperechoic dots, or vascularization from noninfiltrative BCCs. Strain ratios were similar in both infiltrative and noninfiltrative BCCs (mean ± SD, 1.82 ± 0.879 versus 2.2 ± 1.11). However, infiltrative BCCs had statistically increased marginal stiffness in comparison with noninfiltrative BCCs (88.0% versus 18.8%). Increased marginal stiffness had sensitivity and specificity of 0.89 and 0.82 respectively, with a positive predictive value of 0.67 for infiltrative BCCs and a negative predictive value of 0.95 for noninfiltrative BCCs.Conclusions-Histologic variants of BCC have different elastographic patterns. These differences may be of help in preoperative assessments of the BCC subtype and specific surgical planning, avoiding unnecessary skin biopsies.
